How much do you get per captured city, refinery, and factory per turn?

Last I checked, I believe they told you in the Tutorial.
Of course, I could be mistaken.

IF you go into your main menu (not the load/save one) - the one with jagged looking bars with a unit overview, facility overview, march all command, etc - and bring up the facility overview, it will show you all facilities that you own, including how much money and oil it generates per turn.

Thanks fellas I couldn't find it in the manual and didn't bother to play the tuttorial. Now if you'll just tell me if I'm correct in assuming that repairs to damaged units cost a percentage of the total value of the unit.

Hmm, not sure about that one. I'm going to be playing it today, so I'll keep my eyes peeled.

I also can't say for sure, although I do remember seeing my funds go down after replacing lost subunits. I even remember that I was not able to replace subunits due to lack of funds at some point.
However, I am not sure if it takes funds to replace HP on subunits.

It costs funds to repair units (sub units too).
I'm not sure of the exact percentage but I don't think it's a whole number. It cost 62 to replace 6 points of damage to a Leopart (440$). If it was dollar for dollar then it should have come to $66.
That was just a first test though...
As to why I've been away: gonna be moving soon to a bigger house in Portland. Gotta get a room for the little one. Anyway, been organizing stuff for the move.
Cheers.

It costs $$$ to re-arm weapons also. Those atcms (big rockets) was like $200 just for the ammo.

I wish they provided more info on the mechanics of how your resources are spent so I could plan a little better. Isn't this stuff listed somewhere?

All that info is on pages 56-74 in the manual. You know, the pages that got cut out of it.

That $200 bucks is coming as a surprise though. The manual isn't very specific about alot of things in the game.
