Be specific, is the disc scratched, dirty, what? How does it act when in the console?

its not scratch but when i put it on the console it makes a weird sound and then it says that the disk is damage.

If you're not getting similar errors with other discs I would suggest bringing the game back for an even exchange!
If it is happening with other discs also either get a lens cleaning kit or consider changing the disc drive! It may sound a bit drastic but I was getting similar errors with many discs and the change was pretty easy!
