Not for North America, yet. It is in production and should be out soon. There was a delay in the NA release of both console and pc due to the hurricanes in the south east. Keep your eyes peeled.

Yager is now out for Xbox. PC will be out in the next couple of months.

I have a question about the different platforms. When games are created for multiple platforms, i.e. Xbox and PC, how much work is involved to port a game from one platform to another? Is it just a matter of writing different code? And do the developers often make changes in the game when porting it?

It really depends. Some systems are easier to port from PC than others for instance. And, since there is a difference in PC gamers and console gamers, often times games are changed to reflect this. Developers and publishers often times change games, even from console to console. Also, the budget which a game has for development is a big factor. I hope this answers some of your questions.
